\nSchedule: Monday–Friday, 9:00 AM–5:00 PM
\nWeekend Coverage: Some weekend coverage required

\n \nThis role provides psychosocial support to hospice patients and their families across home care, nursing home, and inpatient settings . The Social Worker will be an integral member of the interdisciplinary hospice team, developing individualized plans of care, supporting patients and families through the hospice and bereavement process, and coordinating with clinical and facility staff.
